Social Worker

Reports To: 
Director of Residential Services


Under the general supervision of the Director of Residential Services, the Social Worker is responsible for collaborating with the treatment team in providing case management, documentation, assessment and therapeutic services to Maryvale’s residential clients and their families, as well as coordinating treatment services with the Department of Children and Family Services and Juvenile Probation.

 

ESSENTIAL JOB DUTIES:

 

  1. Demonstrate a passion and commitment for the Vision and Mission of Maryvale and the Mission and Vincentian Core Values of Maryvale and the Daughters of Charity.
  2. Promote and support the attitudes, behaviors, knowledge and skills necessary to work respectfully and effectively with each other as well as demonstrate the ability to address the needs of the children and families Maryvale serves.
  3. Conduct initial assessment of clients and families to determine treatment services needed. This includes administering psychological tests during the assessment process.
  4. Collaborate with members of the multi-disciplinary treatment team for the delivery of a full spectrum of clinical services.  Responsible for coordinating family involvement in treatment Communicate with parents and other approved family members. Encourage and facilitate a family environment in parent education, weekly visits, family therapy, and other Maryvale events.
  5. Coordinate case conferences, discharge planning meetings, and other meetings as assigned to determine the case management needs of clients and their families.
  6. Responsible for assisting in the development of comprehensive treatment plans. Assist clients in meeting individual treatment plans, goals and objectives for assigned cases.
  7. Responsible for conducting mental health therapy groups on various topics such as life skill development, vocational training, anger management, trauma and grief, and other topics relevant to the Residential Treatment Program. 
  8. Responsible for documenting progress through completion of Department of Mental Health documentation and providing feedback to treatment team members. Complete and maintain all documentation of services delivered which includes individual records, group notes and general communication logs.
  9. Act as a liaison between Maryvale and Educational and medical personnel to ensure quality medical and educational services are provided to the client.
  10. Conduct case management services at Maryvale’s Residential Program as needed.
  11. Attend all appropriate DCFS County meetings and in-service trainings. Interface with referral agencies such as Department of Children and Family Services, Juvenile Probation, and Department of Mental Health. Complete and submit Needs and Services Plans, Quarterly Reports, and Discharge Plans on a timely basis.
  12. Responsible for coordinating hospitalizations with DCFS, Juvenile Probation, and treatment staff.
  13. Provide individual counseling services as needed.
  14. Responsible for attending all required Maryvale weekly and monthly meetings.
  15. Perform other duties as assigned by the Director of Residential Services.

QUALIFICATIONS:

  1. Master’s Degree in Social Work, Psychology, Counseling, or a related field. 
  2.  Four years experience in a mental health setting. 
  3.  Must be registered with the BBS or BOP.
  4. Must be proficient in Microsoft Office products, including Word, Excel, and Outlook and 45 w.p.m accuracy.
  5. CPR and First Aid trained; Pro-Act trained.
  6. Possess the ability to communicate effectively through oral and written skills.
  7. Possess case management experience working with SED children.
  8. Bilingual (English/Spanish) preferred.
  9. Valid California Driver’s License.
